Effects on HPA-Axis: At therapeutic doses, no significant difference was detected between inhaled ciclesonide and placebo on hypothalamic-pituitary-adrenal HPA ; function and serum cortisol levels as detailed below. Lamictal was restarted for mood stabilization on this admission at 25mg qhs with the plan to titrate the medication to a therapeutic dose. On day 6 of this admission the chlorpromazine was increased to 50mg qid. On day 12 chlorpromazine was changed to a singe 200mg dose at bedtime to resolve complaints of poor sleep. Day 16 lamotrigine was increased to 50mg at bedime. The following day an additional 50mg of chlorpromazine bid was added to the regimen. The patient had complaints of arthralgias throughout her stay, especially in the knees and ankles. Vioxx 25mg daily was started on day 21. Shortly thereafter, the patient broke out with a malar rash with photosensitivity. On day 25 chlorpromazine was increased to 50mg bid and 250mg qhs. On day 27 the bedtime dose was increased to 300mg. On day 28 the Vioxx was discontinued due to the manufacturer withdrawing the product from the market. Relafdn 1000mg daily was initiated. On day 31 lamotrigine was increased to 100mg qhs. Shortly thereafter the patient complained of sores in her mouth and some ulceration looked suspicious. On day 33 a dental consult reported lesions that looked suspicious for lupus. On day 35 FANA & RF were negative and anitihistone was 100, 000. Lamotrigine was discontinued and the rash, arthralgias and photosensitivity resolved almost immediately. The antihistone was repeated on day 60 and was down to 39.0. Zoloft was added to the patient's regimen for symptoms of depression and titrated to 100mg qhs. Patient was eventually discharged to a group home on the above medication regimen. Conclusions Drug-induce lupus is not well understood and clear identification can be difficult, especially in drug regimens with multiple agents that are suspect. It is also difficult because DIL can occur after years of exposure of the causative agent. This makes it more challenging to draw a temporal association. Initially, arthralgias were contributed to obesity and congenital foot deformities. Chlorpromazine was first suspected as the offending agent for the rash and photosensitivity. Even though the elevated antihistone pointed to drug-induced lupus, Lamictal was withdrawn first because of the oral lesions and fear of Stevens Johnson Syndrome. When the symptoms resolved so quickly and the antihistone level dropped so dramatically lamotrigine appeared to be the cause of the DIL.
